About the Role:
We are currently seeking a dedicated and experienced Psychiatric Registered Nurse (RN) to join our home health team on a per diem basis. This field-based role focuses on delivering compassionate mental health nursing care directly in patients' homes. The ideal candidate will be clinically skilled, self-directed, and committed to supporting mental wellness, stability, and independence in the community setting.
Key Responsibilities:

Conduct thorough in-home psychiatric assessments and ongoing evaluations.

Develop personalized care plans in collaboration with physicians and interdisciplinary team members.

Deliver skilled nursing care tailored to each patient’s mental health needs, including education and medication management.

Monitor patient progress, medication response, and behavioral changes; adjust care plans as needed.

Document all visits and communications accurately and promptly in accordance with regulatory and agency guidelines.

Coordinate care with internal clinicians, therapists, and external support services.

Provide guidance and education to patients and caregivers on mental health conditions, medication adherence, and effective coping strategies.

Respond appropriately to psychiatric crises or urgent needs within the RN scope of practice.

Maintain up-to-date knowledge of Medicare guidelines, state regulations, and agency protocols.

Current, active RN license in the state of Florida.

1 years of recent psychiatric or mental health nursing experience (home health).

Prior home health experience.

Strong assessment, clinical judgment, and communication skills.

Reliable transportation, valid driver's license, and auto insurance.

Ability to work independently and manage a flexible per diem schedule.
