Position Overview
We are seeking a highly organized and detail-oriented Intake Coordinator to join our growing team. This role is responsible for managing new patient referrals, coordinating admissions, and ensuring all required documentation and eligibility requirements are met for timely start of care.
Key Responsibilities
- Receive and process new patient referrals for home health services
- Verify insurance eligibility and benefits (Medicare and other payers)
- Coordinate with referral sources, clinicians, and internal teams to ensure timely admissions
- Ensure all required documentation is complete and compliant prior to start of care
- Communicate with patients and families to gather necessary information and provide updates
- Enter and maintain accurate patient information in the EMR system
- Track and follow up on pending referrals and documentation
- Maintain compliance with Medicare guidelines and company policies
- Previous experience in healthcare intake, admissions, or insurance verification (home health experience preferred)
- Knowledge of Medicare guidelines is strongly preferred
- Strong organizational skills and attention to detail
- Ability to multitask and prioritize in a fast-paced environment
- Excellent communication and customer service skills
- Experience with EMR systems is a plus (Kinnser/Wellsky)
