Senior Care Solutions is a growing Medicare-certified home health agency dedicated to delivering exceptional, patient-centered care throughout South Florida. We are seeking an experienced Intake Coordinator II to join our team and play a key role in ensuring patients receive timely, seamless access to home health services.
This position is ideal for an organized, detail-oriented healthcare professional with experience in home health intake, insurance verification, and coordinating admissions in a fast-paced environment.
Position Summary
The Intake Coordinator II is responsible for managing the full intake process from referral receipt through admission. This includes verifying insurance eligibility, obtaining physician orders and required documentation, coordinating admissions, and maintaining communication with referral sources, clinicians, and patients to ensure an efficient and compliant intake process.
Responsibilities
- Manage the complete referral and intake process from referral acceptance through patient admission.
- Verify Medicare, commercial insurance, and other payer eligibility and benefits.
- Obtain physician orders, face-to-face documentation, and all required clinical documentation.
- Review referrals for accuracy, completeness, and Medicare compliance.
- Coordinate Start of Care scheduling with Clinical Managers and field clinicians.
- Communicate with hospitals, physician offices, case managers, discharge planners, and referral partners regarding referral status.
- Collaborate with the Intake Coordinator I team to ensure referrals are processed efficiently and prioritized appropriately.
- Work closely with scheduling, clinical leadership, and field staff to facilitate timely patient admissions.
- Maintain accurate and timely documentation within the electronic medical record (EMR).
- Identify and resolve issues that may delay admissions while escalating concerns when appropriate.
- Ensure compliance with Medicare Conditions of Participation, agency policies, HIPAA, and all regulatory requirements.
- Assist with training and mentoring new Intake Coordinators as needed.
- Perform additional intake and administrative duties assigned by leadership.
- High school diploma or equivalent required; Associate's or Bachelor's degree preferred.
- 1 year of home health intake, admissions, referral management, or related healthcare coordination experience.
- Medicare Home Health experience strongly preferred.
- Experience verifying insurance benefits and obtaining physician orders.
- Knowledge of Medicare regulations and home health documentation requirements.
- Excellent communication, customer service, and relationship-building skills.
- Strong organizational skills with the ability to manage multiple referrals simultaneously.
- Proficient in Microsoft Office and electronic medical record (EMR) systems.
- Experience with WellSky/Kinnser is highly preferred.
