Position Overview
The Clinical Team Support Specialist / Scheduler is a key operational role responsible for coordinating clinician schedules, supporting daily clinical operations, and serving as a central communication hub between clinicians, leadership, and administrative teams. This position requires exceptional organization, adaptability, and a proactive mindset in a fast-paced healthcare environment.

What You'll Do
Scheduling & Clinical Coordination
- Create, manage, and maintain daily and ongoing schedules for clinicians (RNs, LPNs, PTs, PTAs, OTs, COTAs, SLPs, MSWs)
- Coordinate visits in accordance with physician orders, patient needs, and Medicare requirements
- Proactively manage coverage for new admissions, call-outs, reschedules, and changes in patient status
- Communicate schedule updates clearly and promptly to clinicians and leadership
- Monitor visit utilization, frequencies, and productivity to support operational goals
Clinical Team Support
- Serve as a primary point of contact for clinicians regarding visit assignments and scheduling support
- Partner closely with Clinical Managers, Intake, QA, and Operations to ensure continuity of care
- Support onboarding of new clinicians by assisting with initial schedules and orientation coordination
- Provide day-to-day administrative support to clinical leadership
Documentation & Compliance Support
- Ensure scheduling aligns with physician orders, plans of care, and Medicare guidelines
- Track missed visits, rescheduled visits, and required follow-ups
- Maintain accurate, timely documentation within the EMR system
- Support compliance with agency policies, procedures, and regulatory standards
Administrative & Operational Support
- Respond to phones, emails, and internal communications related to clinical operations
- Assist with reports, tracking logs, audits, and special projects
- Perform additional duties as assigned by management to support agency operations and growth

What We're Looking For
- High school diploma or equivalent required; healthcare, scheduling, or administrative experience preferred
- Prior home health scheduling experience strongly preferred
- Working knowledge of Medicare home health workflows a plus
- Exceptional organizational and time-management skills
- Strong written and verbal communication skills
- Ability to multitask, problem-solve, and remain calm in a fast-paced environment
