PRN MSW – Home Health
Position Summary:
We are seeking a compassionate and resourceful MSW to join our Home Health team on a PRN basis. In this role, you will provide in-home support to patients and families, helping them navigate the emotional, social, and environmental challenges related to illness, aging, and recovery.
As a PRN MSW, you’ll work collaboratively with an interdisciplinary team to support patients’ well-being, promote independence, and connect them with the resources needed to thrive in the home setting.
Responsibilities:

Conduct initialassessments to evaluate the emotional, social, and environmental needs of patients.

Develop and implement individualized care plans in collaboration with the interdisciplinary team.

Provide supportive counseling to patients and caregivers coping with illness, disability, grief, or life changes.

Assist patients and families with accessing community resources, including financial aid, transportation, mental health services, and long-term care planning.

Participate in discharge planning and care coordination activities.

Document all services provided accurately and timely in WellSky.

Maintain compliance with federal, state, and agency regulations and standards of care.

Collaborate closely with nurses, therapists, physicians, and other care team members.

Current FL licensure
-
Minimum 1 year of clinical experience; home health or healthcare setting preferred.

Working knowledge of community resources, entitlement programs, and care coordination practices.

Proficiency with electronic medical records preferred. (Kinnser Wellsky)

Strong communication, problem-solving, and interpersonal skills.

Reliable transportation, valid driver’s license, and auto insurance.

Ability to work independently and manage a flexible field-based schedule.
