Senior Care Solutions is a Medicare-certified home health agency committed to providing exceptional patient care throughout South Florida. We are seeking a detail-oriented and motivated Intake Coordinator I to join our Intake Department.

This entry-level intake position is ideal for someone who thrives in a fast-paced environment, has excellent organizational skills, and enjoys being the first point of contact in the patient referral process. The Intake Coordinator I plays a critical role in ensuring referrals are received, reviewed, and routed efficiently so patients can begin receiving care as quickly as possible.

The Intake Coordinator I is responsible for monitoring incoming referrals, responding promptly to referral sources, performing initial referral review, verifying basic patient information, and assigning referrals to the appropriate Intake Coordinator for processing. This role serves as the front line of the intake department and helps ensure timely communication with hospitals, physicians, and referral partners.

• Monitor the centralized intake inbox for new referrals throughout the day.
• Respond promptly to incoming hospital and physician referrals.
• Review referrals for completeness and verify initial eligibility requirements (MECCA).
• Assign referrals to the appropriate Intake Coordinator for processing.
• Communicate professionally with hospitals, physician offices, case managers, and referral sources.
• Maintain accurate documentation and referral records within the EMR.
• Prioritize urgent referrals to ensure timely patient admissions.
• Escalate incomplete or high-priority referrals to leadership as needed.
• Maintain confidentiality and comply with HIPAA regulations.
• Assist with other intake and administrative duties as assigned.

• High school diploma or equivalent required.
• Home Health, Hospice, or Medicare experience.
• Strong organizational and multitasking abilities.
• Excellent written and verbal communication skills.
• Ability to work efficiently in a fast-paced environment while maintaining accuracy.
• Strong computer skills, including Microsoft Office.
• Experience with WellSky/Kinnser or other EMR systems is preferred.
