Senior Care Solutions is seeking a compassionate, detail-oriented Baylor Intake Coordinator to join our growing home health team. In this role, you'll help ensure patients receive timely, high-quality care by coordinating referrals, processing admissions, and collaborating with our clinical teams throughout the weekend.

Responsibilities
- Process and coordinate new home health referrals and admissions.
- Verify patient information and ensure all required documentation is complete.
- Communicate with physicians, hospitals, case managers, and referral sources.
- Coordinate with clinical staff to facilitate timely Start of Care visits.
- Maintain accurate patient records in the EMR system.
- Monitor referral status and ensure compliance with Medicare and agency guidelines.
- Provide exceptional customer service to patients, families, and referral partners.
- Collaborate with the weekday intake team to ensure seamless continuity of care.
- Previous intake, scheduling, or healthcare administrative experience.
- 1 years of Home health/Medicare experience.
- Strong organizational and multitasking skills.
- Excellent verbal and written communication.
- Proficiency with Microsoft Office; EMR experience preferred (WellSky/Kinnser experience is a plus).
- Ability to work independently while managing multiple priorities.
- Detail-oriented with strong problem-solving skills.
