Position: Occupational Therapist

Schedule: Monday - Friday, 8:00 AM - 4:30 PM

Location: Senior Care Partners P.A.C.E, Portage

No Nights, No Weekends, No Holidays, No On-Call Days

The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Primary Purpose

Assess, recommend, and provide therapeutic activities, compensatory strategies, determination of home care services, education, environmental modifications or adaptive aids to develop, improve, recover, or maintain basic ADL and IADL roles/responsibilities needed to continue living within the community.

Accountability Chart Key Roles

  • Comprehensive assessments in the home and care planning.
  • Provide treatment to improve/maintain ADL and IADL function.
  • Provide education to participants, caregivers, and AFC/SNF
  • Provide professional supervision of COTA


Duties And Responsibilities

  • Conducts initial and periodic assessments in participants’ home, identifies, and develops a participant-centered plan of care for the home care and functional needs of the participant that follows the organizational mission.
  • Assesses the medical necessity for adaptive and/or durable medical equipment, followed by recommendations to the Interdisciplinary Team (IDT).
  • Continually assesses the need for occupational therapy and home care services/interventions during team discussions and communicates to IDT.
  • Plans, develops, and implements a participant-centered plan for therapy to include I/ADL training, therapeutic exercises, DME education, caregiver education and environmental modification. Actively participates in participant centered care planning.
  • Provides training to caregivers, family members, and friends related to the functional and cognitive abilities of participants that follows the participants goals and the organizational mission.
  • Consults and assists with the Recreational Therapy department in provision of wellness programming groups at the center.
  • Participates in the transition of participants from the hospital, assisted living, and/or skilled nursing facilities and makes recommendations to IDT.
  • Assists in the training of and provides professional supervision of occupational therapy assistants (COTA) and other staff who are implementing a treatment program. Provides ongoing education to COTAs.
  • Adheres to Senior Care Partners PACE policies and occupational therapy practice laws on COTA practice and supervision.
  • Functions as a member of the IDT, communicates participant changes to team members, participates in IDT meetings as directed, and the development of the participant plan of care.
  • Participates in Quality Improvement activities.
  • Documents in the participant’s medical record as required by policy.
  • Must care, support, and collaborate with everyone involved in participant's care including the participant’s family using the patient-family centered care approach.
  • Initiates and/or participates in the development of new programming for the benefit of Participants and that correlates with the organization’s strategic plan, vision and values.


Qualifications

  • Doctorate or Master’s degree from an accredited program of Occupational Therapy required; unless previously grandfathered in under a Bachelor’s degree in Occupational Therapy.
  • Must be a graduate of an Occupational Therapy (OT) Program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE®) or equivalent organizations.
  • Must possess a current, unencumbered, active certification/license to practice as an Occupational Therapist in the State of Michigan.
  • Minimum of one year of experience working with the frail, elderly, or long-term care population.
  • Ability to collaborate effectively with other IDT members.


Minimum Working Requirements

  • Must have reliable transportation, current valid Michigan driver’s license and car insurance.
  • Must be 18 years of age or older.
  • Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19 and Hepatitis B.
  • Must be willing to work beyond normal hours and be part of our on-call rotation.
