What You’ll Do
Lead Daily Home Care Operations
- Oversee and organize PCA workflow to ensure consistent, high-quality participant care.
- Assign daily tasks, breaks, and mealtimes for PCAs.
- Provide coverage for PCA responsibilities when needed and troubleshoot issues in the field.
- Coordinate delivery of supplies in collaboration with the IDT.
- Order and manage department supplies.
- Conduct supervisory visits to participant homes to ensure care standards are met.
- Support new and prospective participants as they acclimate to P.A.C.E. services.
- Ensure accurate and timely care documentation in the EMR.
- Deliver start-of-care information to participants and caregivers.
- Represent PCAs at IDT and PCT meetings.
- Enter meeting notes into the EMR and support care planning processes.
- Collaborate with all departments to ensure a seamless, participant-centered care experience.
- Provide leadership to the PCA team in home care settings.
- Support staff with coaching, problem-solving, and real-time field assistance.
- Help develop policies, procedures, and protocols that strengthen home care delivery.
- Manage payroll for direct reports and ensure timely training and compliance.
- Offer evening and weekend on-call support when assigned.
- Assist during emergency or disaster situations.

Education & Experience
- High school diploma required; associate degree preferred.
- Minimum 2 years of professional leadership experience OR equivalent education and relevant work experience.
- CNA certification or willingness to complete PCA training provided by Senior Care Partners P.A.C.E.
- At least 1 year of experience working with the elderly, frail, or long-term care populations.
- Strong communication skills — written, verbal, and interpersonal.
- Detail-oriented with excellent problem-solving abilities.
- Ability to build team cohesion, coach others, and promote a positive culture.
- Proficient with MS Office (Word, Outlook, PowerPoint, Excel).
- Comfortable working independently and making decisions when needed.
- Patient, tactful, compassionate, and resilient — even in challenging situations.

- CPR certification (or ability to obtain through our training).
- Valid Michigan driver’s license, reliable vehicle, and insurance.
- Must be 18 and fully vaccinated (or willing to become fully vaccinated) per program requirements.
- Ability to work onsite; drive in all weather; and navigate unpredictable home environments.
- Frequent interaction with staff, participants, caregivers, and external partners.
- Occasional extended hours and potential emergency call-backs.
- Ability to push/pull, stand or sit for long periods, and lift up to 20 lbs.
- May encounter infectious materials, varying home conditions, and other environmental factors.
