Position Summary
We are seeking a dedicated Occupational Therapist (OT) to join our rehabilitation team. The OT will assess, plan, and implement therapeutic programs to help residents improve their ability to perform daily living and functional activities. This role supports residents recovering from injury, illness, or managing chronic conditions.
Key Responsibilities
- Conduct thorough occupational therapy evaluations to assess physical, cognitive, and functional abilities.
- Develop and implement individualized treatment plans focused on ADLs, strength, coordination, safety, and functional performance.
- Provide direct therapy services including therapeutic exercises, functional task training, adaptive equipment training, and environmental modification.
- Document evaluations, progress notes, and discharge summaries according to regulatory and facility standards.
- Collaborate closely with PT, SLP, Nursing, and Social Services to ensure an integrated, resident-centered plan of care.
- Educate residents, families, and staff on techniques for safe mobility, self-care activities, and the use of assistive devices.
- Participate in interdisciplinary care plan meetings and make appropriate professional recommendations.
- Maintain compliance with federal, state, and facility requirements and uphold best practices in occupational therapy.
Qualifications
- Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program.
- Current Occupational Therapist license in the state of practice.
- Prior experience in a skilled nursing facility or rehab setting preferred.
- Strong clinical assessment, documentation, and communication skills.
- Ability to work collaboratively in a team-focused environment.
