Key Responsibilities
- Coordinate and manage discharge planning for patients in an acute inpatient setting
- Collaborate with physicians, nurses, and interdisciplinary teams to ensure safe and timely patient transitions
- Assess patient needs, develop care plans, and connect patients with appropriate community resources
- Provide case management support, including coverage for Emergency Department (ED) during later shifts
- Ensure compliance with hospital policies, regulatory requirements, and documentation standards
- Maintain accurate, timely, and detailed patient records
- Communicate effectively with patients, families, and healthcare teams
- Master of Social Work (MSW) β Required
- Active LMSW license in the State of Michigan β Required
- Minimum 5 years of case management experience
- Minimum 2 years of recent acute inpatient case management experience (REQUIRED)
- Strong experience with discharge planning

Work Schedule
- 8-hour shifts:
- 8:00 AM β 4:30 PM
- 3:00 PM β 11:30 PM (primarily ED support)
- 10-hour shifts:
- 3:00 PM β 1:30 AM (ED support)
- Weekend rotation required
- No holiday shifts
Salary : $50 - $58
