Licensed Hospice Social Worker (LBSW)
Victoria, TX | Full-Time | Salary: $78K - $80K DOE.

What You'll Do
As a valued member of our interdisciplinary hospice team, you'll:
- Provide emotional support and counseling to patients and families navigating end-of-life care.
- Complete psychosocial assessments and develop individualized plans to meet each patient's unique needs.
- Connect patients and families with community resources, advance care planning, financial resources, and supportive services.
- Serve as an advocate, educator, and trusted guide throughout the hospice journey.
- Collaborate with nurses, physicians, chaplains, aides, and other team members to provide exceptional, holistic care.
What You'll Bring
Required Qualifications:
- Master of Social Work (MSW) from a CSWE-accredited program.
- Current Texas Social Work license (LMSW or LCSW required).
- Minimum of one (1) year of healthcare experience (hospice or home health preferred).
- Strong communication, organization, and relationship-building skills.
- Current CPR certification (or willingness to obtain).
- Valid driver's license, reliable transportation, and automobile insurance.
