MDS Assistant RN/LPN

Scenic Wellness Nursing & Rehabilitation is hiring an experienced MDS Assistant to join our skilled nursing team in Herculaneum, MO. The MDS Assistant supports the Director of Nursing (DON) and RN Assessment Coordinator/MDS Coordinator with the Minimum Data Set (MDS), Resident Assessment Instrument (RAI), care planning, documentation, and regulatory compliance processes.

This is a great opportunity for an RN or LPN/LVN with skilled nursing experience who is detail-oriented, organized, and knowledgeable about MDS, Medicare, Medicaid, and long-term care regulations.

MDS Assistant Responsibilities

• Assist with the MDS and RAI process from assessment through interdisciplinary care plan completion
• Assist with ensuring compliance with Federal and State regulations and long-term care certification requirements
• Coordinate and monitor MDS assessments, RAI tracking, discharge/re-entry, and Medicaid tracking
• Enter and maintain MDS and assessment information in PointClickCare
• Ensure timely and accurate MDS submissions to the State
• Follow up on MDS submission validation errors and maintain accurate validation records
• Assist with determining the Assessment Reference Date (ARD) based on resident care needs
• Complete resident assessments, data collection, and interviews with staff as needed
• Review clinical documentation and follow up with staff when additional documentation or clarification is needed
• Assist with maintaining accurate and compliant care plans and clinical documentation
• Monitor documentation for compliance with established standards of practice
• Assist with regulatory and certification surveys and correction of deficiencies
• Identify documentation trends and report audit findings to the Quality Assurance and Performance Improvement (QAPI) Committee
• Work collaboratively with nursing, therapy, dietary, social services, and other interdisciplinary team members
  
  
  

Qualifications

• Current RN or LPN/LVN license in the state of Missouri
• Previous Skilled Nursing Facility (SNF), long-term care, or nursing home experience preferred
• MDS, RAI, or MDS Coordinator experience preferred
• Knowledge of Minimum Data Set (MDS), RAI, OBRA regulations, and care planning
• Knowledge of Medicare PPS, Case-Mix, and Medicaid reimbursement preferred
• Understanding of Quality Measures/Quality Indicators and clinical documentation
• Experience with PointClickCare preferred
• Strong attention to detail and organizational skills
• Ability to prioritize multiple deadlines and maintain accurate records
• Strong communication and follow-up skills
• Ability to work effectively with an interdisciplinary team
