Key Responsibilities:
- Develop and update individualized support plans in collaboration with clients and families
- Coordinate and monitor service delivery across multiple providers
- Connect clients with community resources, benefits, and support programs
- Maintain accurate and timely documentation in compliance with MDHHS/County regulations
- Advocate for client needs and preferences
- Respond to emerging needs and crisis situations as required
Qualifications:
- Bachelor's degree in Human Services, Social Work, Psychology, or related field
- Current MichiCans training
- QIDP certification preferred
- 1 year of experience coordinating care plans and managing community resources
- Valid Michigan driver's license and reliable transportation
- Strong knowledge of Michigan Medicaid waiver programs
Job Types: Full-time, Part-time, Contract
Pay: From $212.00 per day
Expected hours: No less than 37.5 per week
