Job description:
McCrory Center is seeking an experienced, compassionate, and highly motivated Supports Coordinator to join our growing team. The ideal candidate is committed to providing exceptional service, empowering individuals and families, and ensuring clients receive the resources, services, and supports they are entitled to.
This position requires frequent interaction with clients in the community, in their homes, and in our offices. The successful candidate must be dependable, organized, professional, and dedicated to delivering high quality services while maintaining the highest ethical standards.
Responsibilities:
- Coordinate services and supports based on each client's individual needs
- Meet with clients in their homes, in community settings, and in the office
- Connect clients and families with appropriate community resources and benefits
- Ensure clients receive the services and entitlements for which they qualify
- Develop, maintain, and monitor person centered service plans
- Maintain accurate, timely, and professional documentation that meets agency and regulatory standards
- Collaborate with providers, community partners, and family members to promote positive outcomes
- Advocate for clients while maintaining confidentiality and ethical practice
- Respond promptly to client needs and maintain regular communication
- Manage and grow an assigned caseload while maintaining quality service delivery
- Travel locally to complete client visits and attend meetings as needed
Qualifications:
- Bachelor's degree from an accredited college or university (required)
- Previous experience as a Supports Coordinator or Case Manager (required)
- Valid driver's license, reliable transportation, and current automobile insurance (required)
- Excellent verbal and written communication skills
- Strong organizational and time management skills
- Knowledge of community resources and human service systems
- Ability to work independently while collaborating with a multidisciplinary team
- Professional demeanor with a positive, upbeat attitude and a service oriented mindset
