Transitional Specialist

Village Green Health & Rehabilitation is seeking a dynamic and compassionate Transitional Specialist to assist residents and their families with the admission process to the discharge process and beyond. Prior experience in a hospital or long-term care setting is preferred. Candidate should have the ability to prioritize tasks quickly based on need and time sensitivity and be capable of multitasking while remaining organized and courteous. Candidate must have proficient computer skills in a business setting and possess excellent communication and customer service skills.

Duties include:

• Assist admissions with registration paperwork
• Understand insurance and detail to paperwork
• Attend new admission meetings
• Conduct account research and analysis
• Prepare and maintain various reports
• Identify and execute the necessary process adjustments
• Send statements to outstanding customers
• Interact with customers to resolve outstanding issues
• Assist social services with assessments and other daily tasks
• Conduct discharge follow-up calls
• Interact with the Interdisciplinary Team on systemic process improvement
• Involvement in the plan of care process with residents and their responsible parties
• Coordinate with community partners
