Responsibilities
- Administer medications and injections accurately following physician orders.
- Support patients with activities of daily living (ADLs).
- Monitor patients’ conditions closely, recognize signs of acute pain or deterioration, and escalate concerns appropriately.
- Collaborate effectively with the residents, families, physicians, and case managers to develop and implement individualized care plans .
Qualifications
- Valid Licensed Practical Nurse (LPN) license in good standing.
- Strong knowledge of medical terminology and medication administration protocols.
- Demonstrated proficiency with Point Click Care is a plus.
- Excellent communication skills to effectively educate patients and coordinate with healthcare teams while maintaining strict confidentiality policies under HIPAA standards.
