Description
Country Village Center seeks a Licensed Practical Nurse to deliver resident-centered care in our skilled nursing and rehab facility in Maine. The LPN will provide direct clinical care, including medication administration, treatments, vital signs monitoring, wound care support, and documentation in EMR. You will collaborate with an interdisciplinary team to support individualized care plans, promote safety and infection control, and build strong, respectful relationships with residents and families in a compassionate, team-oriented environment. Administer medications and treatments per care plan and facility policy
Monitor, document, and report changes in resident condition using EMR systems
Support care planning and participate in interdisciplinary team meetings
Assist with wound care, respiratory care, and rehab-focused interventions
Educate residents and families on care plans and health management
Maintain infection control, safety, and regulatory compliance standards
Support a resident-centered culture of dignity, respect, and compassion
