The Registered Nurse (RN) Case Manager for Home Care contributes to our Mission of Caring by providing compassionate direct patient care to our clients in a one-on-one setting in their homes. The Registered Nurse (RN) Case Manager for Home Care conducts day-to-day nursing activities to include assessment and education. The Registered Nurse (RN) Case Manager for Home Care for Saint Elizabeth Home Care is a part of a cohesive and collaborative interdisciplinary team with ample administrative support. Our Registered Nurse (RN) Case Manager for Home Care are equipped with point of care devices (iPad), as well as all necessary and requested medical equipment and supplies.
Qualifications for a Registered Nurse (RN) Case Manager for Home Care:
- Current unencumbered RN license in Rhode Island.
- Minimum of one year nursing experience, home care experience preferred.
- CPR certification.
- Reliable transportation, current clean driver's license, automobile insurance.
