SUPERVISOR CFTSS/HCBS

Job Summary: The Supervisor in CFTSS/ HCBS is responsible for reviewing and approving progress notes for Home and Community-Based Services (HCBS) and Children and Family Treatment and Support Services (CFTSS). This role ensures that documentation accurately reflects the scope, quality, and effectiveness of services provided to clients. The Supervisor will verify that all progress notes meet regulatory and compliance standards while also supporting the integrity and efficiency of clinical service delivery. In addition, will provide supervision and guidance to staff, fostering professional development and promoting excellence in documentation practices.
Duties & Responsibilities
Review and approve progress notes to ensure accuracy, completeness, and adherence to standard documentation guidelines. Confirm that progress notes include all required demographic information for each child/youth. Verify that progress notes clearly identify the type of contact, modality, and location where services were delivered. Ensure documentation includes the date of service and identifies all participants present during the session. Evaluate the quality of interventions documented and assess the response of the child/youth and family/caregiver. Ensure progress notes accurately reflect the goals and objectives addressed during the session, including the client’s progress toward these goals. Notify the VP/AVP of any incidents referenced in staff documentation and complete incident reports as needed. Provide supervision to staff related to caseload management, service delivery, client progress, and development of Plans of Action to support goal attainment. Maintain supervision records and ensure staff compliance with licensure, training, and documentation standards. Work closely with the Electronic Health Record (EHR) system to ensure compliance with documentation and billing requirements. Ensure that direct-hired staff schedules, service time, and documentation are in compliance with ADP and timekeeping requirements. Attend and actively participate in clinical review meetings, providing guidance and recommendations to support program quality. Ensure that at least 80% of direct-hire workers’ time is allocated to direct service delivery. Lead and execute audit reviews with all service providers to identify risks, ensure data accuracy, and maintain regulatory compliance. Offer consistent monthly supervision to assigned team members, fostering professional growth, accountability, and high-quality service delivery. Provide feedback to program management regarding staff whose documentation does not meet quality standards and requires correction. Participate in bi-weekly in-person supervision sessions with staff. Conduct in-person onboarding training for new staff. Perform additional duties as assigned by the supervisor. Ensure completion of all supervisory responsibilities, including follow-up on mandatory training requirements, timely performance evaluations, and ongoing staff support.
Education & Experience
One of the following NYS Professional License is required: LCSW /LMSW/ LMFT/ LMHC. Two (2) years of related experience and 1 year of supervisory experience is required.

While performing the duties for this job, the employee is regularly required to communicate clearly and effectively. The employee is required to stand, use mobility, handle, feel; and reach. The employee is occasionally required to sit and stoop, kneel or crouch. The employee must occasionally lift and/or move up to 25 pounds.
