Qualifications and Competencies
- Master’s Degree in Human Services with license/certification (SW, CASAC, CAT, OT, Psychologist, etc.).
- 3 years human services experience; mobile team preferred.
- 1 year specialty experience preferred.
General Description
Provides direct clinical care, integrated treatment, rehabilitation, and support services to ACT recipients with mental illness and substance use conditions.
Duties and Responsibilities
- Delivers face-to-face specialty services in the community.
- Develops treatment and recovery plans with trauma-informed care.
- Provides counseling, wellness, and relapse prevention supports.
- Utilizes harm reduction, CBT, motivational interviewing, and family support.
- Facilitates individual and group educational sessions.
- Supports vocational goals, employment counseling, and entitlements.
- Participates in on-call crisis rotation and daily team meetings.
- Documents all services and outcomes in compliance with ACT guidelines.
- Accepts and follows through on other duties and responsibilities as assigned.
Physical Demands & Work Conditions
- Travel to client sites and agency locations required.
- Staggered schedule (8AM–8PM) with on-call duties.
- Direct, face-to-face work with diverse clients required.
