Qualifications and Competencies
- High School Diploma 5 years human services OR Bachelor’s degree.
- Organizational and communication skills for working with staff, clients, families, and community.
- Critical thinking and judgment skills required.
General Description
Provide supportive counseling and assistance training individuals diagnosed with mental illness in independent living skills. Promote greater independence in day-to-day activities, personal care, skill building, and community integration.
Duties and Responsibilities
- Participates in planning and evaluation of service plans and monitors objectives.
- Writes progress notes monthly or as needed.
- Trains and assists residents in daily living skills.
- Monitors medication.
- Provides transportation and accompanies residents to activities.
- Offers counseling, crisis management, and behavioral interventions.
- Assists in daily management of the residence.
- Administers residence in absence of Senior Counselor.
- Documents emergencies and notifies supervisors as needed.
- Case management, follow-up, and other duties as assigned.
Physical Demands & Work Conditions
- Adheres to HIPAA compliance.
- Valid driver’s license and car with insurance.
- Completion of mandatory trainings.
- Fingerprint/background check required.
