POSITION TITLE: Child Case Manager Supervisor

RESPONSIBLE TO: Clinical Director

DEPARTMENT/SITE: SCCMHA / Saginaw

STATUS: Exempt; salaried. Unit requirement.

All the criteria-based duties and standards within this document will be performed according to established policies, procedures and guidelines within the department/site and SPSI.

JOB SUMMARY

The child case manager supervisor is the principal provider of direct care clinical service at Saginaw Psychological Services. It is the responsibility of the supervisor to provide guidance, counsel along with follow through on specific work-related functions as directed in your position. The Supervisor works in the agency’s Mental Health Counseling center and may have a caseload of clients with varying diagnosis which none will be discriminated against.

ESSENTIAL JOB DUTIES

SUPERVISOR

1. Provide individual supervision to employee at least once per month.

2. Schedule and complete orientations as needed.

3. Assign cases to the SCCMHA team and reassign cases when needed by following SPSI internal procedure (see procedure) and also changing the admission in EMR Systems.

4. Track unit requirements for the SCCMHA team, meet with case managers if they are not meeting their required unit averages and write up progressive discipline according to the agreement.

5. Complete in-services for case managers on repeated issues that the SCCMHA team lead has identified as concerns.

6. Assist and attend annual audits required by SCCMHA.

7. Review rosters completed by team and ensure case managers are completing documentation on time. If not, meeting with them to address these issues in supervision or more frequently if needed and writing up progressive discipline if needed (PAF).

8. Review and conduct interviews for the SCCMHA team and complete hiring process with human resources.

9. Ensure emails and phone calls are being addressed and returned in a timely manner (24 hours preferred) due to SCCMHA urgent needs.

10. Ensure availability for crisis or urgent situations that may arise.

11. Respond to therapists, medical provider or nursing concerns and case reassignments in a timely matter to ensure continuity of care of the consumer.

12. Attend monthly provider meetings at SCCMHA.

13. Attend supervisor meeting and individual supervision once a week unless agreed on with supervisor.

14. Coordinate with SCCMHA team lead and run monthly SCCMHA team meetings.

15. Ensure cases that have not been seen in 30 days are being followed up on.

16. Reviews/signs off on and/or send corrections for assessments, IPOS, annual psychosocial, and periodic reviews or other required documents SCCMHA.

17. Complete annual performance appraisals as they are due.

18. Review pre-screens notification emails and ensure case managers are following up within 7 days of hospital discharge per SCCMHA policy.

19. Ensure all tasks from upper management are being attended to in a timely manner.

20. Ensure to follow discharge procedure with appeals notices and discharges in EMR systems.

21. Will openly acknowledge difficulty in managing any case and seek support and supervision from the director in that department.

22. Will immediately report critical issues and work with their direct supervisor to develop an intervention plan.

23. Will support and attend where possible agency events or initiative, and the goals of the agency strategic plan.

24. Will carry selected and additional administrative assignments as required, case record review, committee assignments, etc.

25. Supervisor will be expected to have an established caseload as determined by their direct supervisor.

26. Coordinating with different professionals and agencies that serve our consumers.

27. Monitoring treatment and medications.

28. Advocating for our consumers to ensure equality and reduce the stigma of mental health/substance use.

29. Respond to routine and emergency contacts from clients and/or staff.

30. Prepare and maintain records and reports, such as budgets, personnel records, or training manuals.

31. Participate in the determination of organizational policies regarding such issues as participant eligibility, program requirements, and program benefits.

32. Research and analyze member or community needs to determine program directions and goals.

33. Act as consultants to agency staff and other community programs regarding the interpretation of program-related regulations and policies.

34. Run productivity and manage invoices from case management department.

35. Maintain your credentials and required trainings to satisfy your licensure and/or insurance otherwise immediate suspension of employment will be needed.

36. Maintain valid car insurance, driver’s license, and car registration always otherwise immediate suspension of employment will be needed.

37. Other duties required as assigned.

CLINICAL THERAPIST

1. Demonstrate a knowledge of assessment techniques including historical assessment, structured interview, and behavioral assessment that contributes to an accurate diagnosis.

2. Work with the client and the treatment team to determine an appropriate course of treatment to meet the client’s goals and the expected outcomes of the case.

3. Demonstrate basic clinical skills including, but not limited to: a capacity to establish and maintain a treatment alliance, and the ability to provide respect and empathy toward the client.

4. Effectively manage counter-transference issues and feelings that might threaten to interfere with treatment.

5. Utilize supervision - reporting progress, success, and difficulty in treating cases assigned to you.

6. Maintain confidentiality of records relating to clients' treatment.

7. Encourage clients to express their feelings and discuss what is happening in their lives, helping them to develop insight into themselves or their relationships.

8. Assess patients for risk of suicide.

9. Prepare and maintain all required treatment records and reports.

10. Guide clients in the development of skills or strategies for dealing with their problems.

11. Perform crisis interventions with clients.

12. Act as client advocates to coordinate required services or to resolve emergency problems in crisis situations (take answering services calls after hours if needed).

13. Meet with families, probation officers, police, or other interested parties to exchange necessary information during the treatment process.

14. Discuss with individual patients their plans for life after leaving therapy.

15. Monitor clients' use of medications and report to their Family Doctor or Psychiatrist any issues seen.

16. Refer patients, clients, or family members to community resources or to specialists as necessary or when other levels of care are needed beyond the therapist scope.

17. Work to continually improve skills seeking training in areas where needs/goals have been identified.

18. Document progress notes, treatment plans, assessments, evaluations and all other required paperwork in a timely fashion according to policy and procedure.

19. Maintain your credentials and required trainings to satisfy your licensure and/or insurance.

MARGINAL JOB DUTIES

1. Participate in interview process as needed.

2. Develop a new training binder for new employees, training new employees as needed including training CMH therapy team on EHR usage.

3. Establish and maintain relationships with other agencies and organizations in community to meet community needs and to ensure that services are not duplicated.

4. Learn about new developments in counseling by reading professional literature, attending courses and seminars, or establishing and maintaining contact with other colleagues.

5. Evaluate the work of staff to ensure that programs are of appropriate participant eligibility, program requirements, and program benefits.

6. The supervisor will provide assistance and leadership by such means as consulting, teaching or supporting other staff members in conducting their responsibilities and enhancing their professional growth as appropriate, and coordinating activates when assigned to do so.

JOB SPECIFICATIONS

EDUCATION: Must have master’s degree, doctoral degree equivalent in related field areas such as, psychology, professional counselor, social work, CAADC/CADC.

LICENSURE: Valid driver’s license, master level social work license.

EXPERIENCE: A minimum of one to three years’ experience in case management in a mental health or substance use disorder (SUD) setting required.

1. Must possess effective communication skills and act as a representative of SPSI and adhering to its mission, vision, and values. This includes external representation of local agencies.

2. Speak to clients in a respectful and calming manner, even if clients are agitated. Identify and resolve client conflicts/dissatisfaction issues as needed.

3. Engages and develops trusting relationships with clients to enhance successful mental health/substance use outcomes.

4. Continually strives for effective relationships with staff, clients and management.

5. Expect self and co-workers to maintain the highest standards of professional ethics.

6. Create enthusiasm for team, set a model for team work and promote strong working relationships.

PHYSICAL EFFORTS: Medium; must be able to lift up to 50 lbs.

HOURS OF WORK: Full-time, flexible and varied. Some evenings and weekends may be required.

TRAVEL: Reliable transportation is required. Local travel throughout service area is required with possible occasional travel between SPSI locations. Some out of service area travel may also be required. Reimbursement in accordance with SPSI policy.
