Job Title: Case Manager (LBSW)
Job Type: Full-Time
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (some 9:00 AM – 6:00 PM shifts). Occasional evenings, weekends, and holidays may be required.
Position Overview
- We are seeking a dedicated Case Manager (LBSW) to provide case management and care coordination services to individuals with mental illness, substance use disorders, and/or developmental disabilities. This role is responsible for managing caseloads, coordinating services, developing care plans, and ensuring clients receive appropriate resources and support to meet their treatment goals.
Key Responsibilities
- Manage an assigned caseload of individuals receiving behavioral health and/or developmental disability services.
- Conduct assessments and develop individualized service plans based on client needs.
- Provide case management and supports coordination to ensure access to appropriate treatment and community resources.
- Monitor client progress and update care plans as needed to support treatment goals.
- Coordinate services with healthcare providers, community partners, and support systems.
- Maintain accurate, timely, and compliant documentation in accordance with regulatory and organizational standards.
- Participate in team meetings, case reviews, and quality improvement activities.
- Support individuals and families by connecting them with housing, employment, healthcare, and other social services.
- Respond to client needs and assist with crisis intervention when necessary.
Minimum Qualifications
- Education - Bachelor’s degree in Social Work or a related Human Services field such as Psychology, Sociology, Child Development/Family Relations, Occupational Therapy, Community Mental Health, Chemical Dependence, Physical Therapy, Rehabilitation Counseling, or Education.
- Licensure - Michigan Bachelor’s Level Social Worker (LBSW) license required.
- Experience - 2 years of experience working with adults with mental illness, substance use disorders, and/or developmental disabilities & 1 year of direct caseload management providing case management or supports coordination services
