Position Overview

The Case Manager supports individuals with SMI, SED, and/or I/DD by coordinating services that promote independence, community inclusion, and overall well-being. This role involves advocacy, service linkage, and monitoring to ensure continuity of care and prevent service gaps.

Key Responsibilities

• Develop and implement person-centered service plans
• Link individuals to medical, social, educational, and therapeutic resources
• Coordinate and monitor services to achieve successful outcomes
• Provide advocacy and support to reduce barriers to care
• Participate in emergency on-call rotation

Qualifications

• Bachelor’s degree in a human services field
• Minimum one year of experience in mental health
• Certification as QMHP, QIDP, and/or CMHP, or Michigan licensure (limited or full)
• Strong organizational, communication, and problem-solving skills
• Ability to work flexible hours and travel within service area
• Valid Michigan driver’s license, vehicle insurance, and satisfactory driving record
