General Definition of Work:

This position links individuals and families to community resources, assists with basic case management, conducts
assessments, and guides clients through “pathways” to improve access to health and social services. The role also tracks
and documents all pathway steps from start to finish.

In addition, the position provides administrative support by coordinating meetings, preparing materials, taking minutes,
and completing required reports and documents. Work is performed under the moderate supervision of the Community
Health Assistant Director.

Essential Functions:
• Work closely with other members of social services and treatment teams, including therapists, social workers,

case managers, and healthcare professionals to ensure a coordinated and holistic approach to client care.

  • Conducts mental health, substance use, and comprehensive social determinants of health screenings to
determine each client’s specific challenges and strengths related to wellness, harm reduction and recovery.
  • Communicates with clients and community partners (e.g., in-person, email, telephone, etc.) internally and
externally with a high degree of customer service to understand and respond to inquiries or share information in
a timely fashion. Builds trusting relationships and support clients to make healthy choices.
  • Determines client’s eligibility for services for financial assistance, insurance and programs, in order to refer clients
to community resources and agencies to meet the identified needs. Provides support in completing pharmacy
assistance program applications, securing transportation for appointments, facilitating transitions in care, and
identifying accessing medical home.
  • Provides health counseling, education and information, social support, and advocacy for patients and families.
Provides peer support and reinforces health education, prevention and disease management. Assists clients in
building individual capacity to address health and social issues.
  • Provides comprehensive, accurate, timely and legible documents and submits reports as required. Utilizes an
Electronic Medical Record (EMR) system to document all client interactions and maintain accurate and
confidential records of client interactions, progress, and outcomes. Assists in orientation and training of newly
hired CHWs as required.
  • Monitors pathway steps through to completion. Follows up with clients when pathway does not progress toward
completion and determines additional intervention/support needed for improvement. Assists in development of
and participation in community outreach activities, health fairs, etc. Serves on-site at clinics, emergency
departments, and collaborating agencies as needed. May make home visits to facilitate pathway completion.
  • Serve as the gateway to Room 312 (CORE) by staffing the front desk, greeting clients, and providing a welcoming,
professional first point of contact. Answer, triage, and address incoming phone calls; assist walk-ins; schedule or
coordinate services; and ensure clients are directed to the appropriate staff or resources.
  • Perform administrative and clerical duties to support program operations, including coordinating and attending
meetings, preparing agendas and materials, transcribing meeting minutes, maintaining program records, and
preparing, processing, and completing required reports and documents.


Minimum Qualifications:
Education: High School diploma or GED.
Experience: Moderate (1 years) experience in a healthcare setting.
Preferred: Previous experience working with Medicaid eligible clients
Special Requirements:

✓ FEMA National Incident Management Systems 100.c, 200.c, 700.b and 800.c within six (6) months of hire.
✓ MiCHWA CHW Training within six (6) months of hire.
✓ Valid State of Michigan Driver’s License upon hire.
