LPN Home Health ($1720 per week gross pay)

We are seeking LPN Home Health for a travel/contract position in Lacey, WA, accepting both local and traveler candidates.Position Details:Position Name: Home Health LPN Location: Lacey, WashingtonSpecialty: Home HealthStart Date: ASAPEmployment Type: Full-time, Travel, Contract, TemporaryContract Length: 13-week contract Pay Rate: Range $1680 to $1720 Weekly Gross pay based on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)Mileage Reimbursement: $0.49/mile for distance covered between patient home appointmentsUnit: Home Health Hospice Shifts available: Days: 8 Hours;Shift timings: Days, 8a – 5p Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri Position Requirements:LPN experience: 1 - 2 years - RequiredRecent/Current LPN Home Health experience: 1 year within the last 3 years – RequiredHome Care Home Base (HCHB) charting - Highly preferredMust have valid Driver’s License and Personal Auto InsuranceEducation, License and Certification Requirements:LPN License for State of the state applying or Compact if acceptable - RequiredBLS Certification from AHA or American Red Cross - RequiredJob Description:Provide skilled nursing care as ordered by the attending physician under RN's supervision.Observe and monitor patient conditions and notify RN case manager of any changes in the patient’s condition.Administer medication as prescribed by the physician under RN's supervision and help decrease re-hospitalizations by educating the patient on preventative measures and making good decisions.Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.Attend IDT Team meetingsTerritories to cover: Lewis county with some crossover, clinical must be flexible with areas of coverage if crossover is required
