(2 admissions/day or routine visits as needed)Performs Case Management, Admissions and all types of visits for the assigned patient case loadSupport patient comfort, pain management and ability to stay at home - enabling them to spend quality time with their loved ones. Empower patients and families by educating them on disease management and pain management.Provide bereavement support.Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.Notify physicians of significant changes in status.Document and complete charting for all patient visits within 24 hours.Provide bereavement support to patient’s families as needed.Territories to cover: Thurston, Lewis and Mason Counties
