Overview:
Safro Staffing is recruiting a dedicated Licensed Practical Nurse (LPN) to join a compassionate, interdisciplinary care team supporting older adults in the Program of All-Inclusive Care for the Elderly (PACE). This full-time position is based in Pittsburgh, PA, and involves delivering both facility-based and in-home care to participants.
PACE is a nationally recognized, community-based program that helps seniors with chronic health needs live safely and independently in their homes and communities. As an LPN in this unique setting, you'll be instrumental in supporting clinical care and improving participant quality of life through proactive, hands-on nursing.
Key Responsibilities:

Monitor and record vital signs and health status

Administer medications and perform wound care

Conduct basic lab work and assist with diagnostic procedures

Support implementation of care plans alongside RNs and providers

Collaborate with the interdisciplinary care team to ensure continuity of care

Travel to participant homes and care facilities as needed
Requirements:

Active Pennsylvania LPN license

CPR and First Aid certification

Minimum 1 year of experience working with elderly or frail adults

Valid driver’s license and reliable vehicle for local travel
