We’re seeking a skilled Home Infusion Pharmacist to join a leading specialty pharmacy team in Montgomery, AL.
This permanent role offers the opportunity to work in an advanced alternate care setting—providing personalized, patient-centered infusion therapy in the home. You’ll play a key role in ensuring safe, effective, and compassionate care for patients receiving complex infusion therapies.
Key Responsibilities:
- Deliver compliant, ethical pharmacy practice aligned with all regulatory and accreditation standards
- Assess therapy appropriateness and patient suitability for home infusion care
- Design and manage individualized care plans with defined therapy goals
- Dispense medications, supplies, and infusion equipment for patient use
- Monitor lab results, perform pharmacokinetic dosing, and document clinical progress
- Educate patients and caregivers on medication administration and infusion techniques
- Ensure USP <797> compliance for sterile compounding and cleanroom operations
- Supervise and delegate responsibilities to support staff, interns, and students
- Participate in interdisciplinary patient care meetings and provide clinical drug information
- Take part in on-call rotation to support patient needs after hours
Qualifications:
- Bachelor of Science in Pharmacy (PharmD or Master’s preferred)
- Active Alabama Pharmacist License in good standing
- Minimum 1 year of recent home infusion experience required
- Strong understanding of IV admixture and infusion therapy
- Excellent communication, critical thinking, and organizational skills
Schedule:
Full-time, Monday–Friday
Typical hours: 8:30 AM–5:00 PM or 9:00 AM–5:30 PM
(Must have flexibility to stay later as needed.)
