This part-time, contract role offers flexible scheduling of approximately 10-20 hours per week, with a guaranteed minimum of 8 paid hours weekly. The position requires residency in California and active enrollment as an Ordering, Referring, and Prescribing (ORP) provider with Medi-Cal.
Key Responsibilities:
- Perform Medication Therapy Management consultations remotely, focusing on optimizing patient medication regimens
- Provide patient counseling and education to promote medication adherence and safety
- Review medication profiles to identify potential drug-related problems and opportunities for therapy optimization
- Accurately document all clinical interventions and patient encounters
- Collaborate with program stakeholders to support patient care goals and improve outcomes
