Full Time Hospice Social Worker (MSW)
Must be able to commute San Gabriel Valley/IE areas.
Salary $35.00-$45.00/hr Mileage Reimbursement
The Social Worker will be an integral part of the Hospice Interdisciplinary Team and will assist the team in understanding the significant social, emotional and cultural factors related to each patient’s terminal illness. The Social Worker conducts psychosocial assessments and evaluations, as well as provide emotional counseling to patients/families in their place of residence – in person and telephonically. The Social Worker works under the direction of a physician and supervision of the Director of Patient Care Services and serves a as resource to our patients, families and our community. Must be able to utilize computers and be comfortable with electronic medical records.

Qualifications
- Master of Social Work degree from a school accredited or approved by the Council on Social Work Education.
- Clinical experience relevant to the psychosocial needs of patients and families.
- Excellent verbal and written communication skills.
- Spanish speaking a plus, but not required.
- Valid driver license and auto insurance and reliable/well maintained transportation.
