The Clinical Care Coordinator (LVN) serves as a liaison between patients, families, referral sources, and the hospice/palliative care team. This field-based role provides education, conducts nursing assessments, coordinates admissions, and helps ensure patients receive timely, compassionate care.
Responsibilities
- Conduct nursing assessments and admissions
- Review consents along with other documents and assists with election of hospice benefit.
- Educate patients and families about hospice and palliative care
- Coordinate care transitions from hospitals and facilities
- Collaborate with interdisciplinary teams
- Complete documentation using EMR systems
- Current California LVN License
- 1 Year Nursing Experience, hospice experience highly preferred
- Strong Communication Skills
- Reliable Transportation & Valid Driver’s License
- Bilingual Spanish Preferred
