Community Outreach and Referral Specialist

Community Partnerships and Referral Specialist responds to information and referral inquiries from seniors, caregivers, and professionals by providing accurate details about PACE services.

Key Duties Include

• Assessing individuals’ needs and coordinating referrals to support enrollment.
• Managing inbound and outbound calls.
• Documenting all interactions and referral activity in a CRM.
• Supporting enrollment efforts and collaborating with marketing to promote services.
• Retrieving and coordinating medical records as needed for enrollment processes.
• Hosting office hours in senior housing settings to build relationships and identify potential participants.
  
  

Qualifications & skills: strong communication and customer service skills, ability to assess needs, accuracy in information sharing, proficiency with CRM documentation, and comfort coordinating referrals and records while maintaining professionalism with seniors and caregivers.
