Key Responsibilities
Clinical Care
- Conduct virtual initiating visits (audio/video) for patients eligible for Chronic Care Management.
- Perform detailed medical histories, medication reviews, and chronic disease assessments.
- Identify care gaps and recommend evidence-based interventions.
- Ensure documentation supports CCM program enrollment according to CMS requirements.
- Collaborate with CCM nursing staff, care coordinators, and behavioral health partners.
- Provide clinical recommendations to guide longitudinal care management.
- Review and co-sign CCM care plans and update them as clinical needs change.
- Accurately document all virtual visits in the EMR.
- Follow CMS guidelines for CCM eligibility, initiating visits, and ongoing patient monitoring.
- Maintain compliance with HIPAA and organizational privacy and quality standards.
Salary : $125 - $150
