JOB SUMMARY:
The Occupational Therapist contracted through our organization is responsible for implementing standards of care for occupational therapy services and adhering to all conditions outlined in the Service Agreement.
DUTIES & RESPONSIBILITIES:
- Assume responsibility for ongoing interdisciplinary assessment and development of individualized care plans in partnership with patients, representatives, and caregivers.
- Provide services as ordered by physicians in the plan of care.
- Offer patient, caregiver, and family counseling.
- Provide patient and caregiver education.
- Communicate with physicians and other healthcare practitioners related to the current home health plan of care.
- Evaluate patients' functional status and home environment for hazards or barriers to independent living.
- Develop treatment programs and establish goals for improved function.
- Train patients in the use of adaptive equipment, prosthetic and/or orthotic devices.
- Maintain appropriate clinical records and report any changes in patients' condition to the physician.
- Participate in quality assessment and performance improvement programs.
- Identify patient and family/caregiver needs for other home health services and make appropriate referrals.
- Instruct patients' family/caregiver and other healthcare personnel in the treatment regime.
- Supervise Certified Occupational Therapy Assistants.
- Instruct, supervise, and evaluate home health aide care when therapy is the only skilled service.
- Participate in discharge planning for patients.
JOB REQUIREMENTS:
- Must be licensed where the HHA is located if licensure is required.
- Must have graduated from an OT program accredited by ACOTE of AOTA or any successor organization.
- Must be eligible for or have taken the OT certification examination administered by NBCOT.
- Two years of clinical experience as an occupational therapist.
- Demonstrates good verbal and written communication and organization skills.
- Possesses and maintains current CPR Certification.
- Must be a licensed driver with an insured automobile in good working order.

Salary : $80,000 - $100,000
