You’ll assess psychosocial needs, provide counseling, and collaborate with an interdisciplinary team to ensure every patient receives holistic, dignified care.

Your Impact
- Comprehensive Assessments: Evaluate psychosocial status of patients and families related to terminal illness and living environment
- Care Planning: Develop and implement interventions based on thorough evaluations
- Counseling & Support: Guide patients and caregivers through stress, coping challenges, and end of life issues
- Documentation: Maintain accurate, timely clinical records for all referred patients
- Resource Navigation: Provide referrals and serve as a liaison with community agencies
- Team Collaboration: Participate in interdisciplinary care planning and regular team meetings
- Advocacy: Help physicians and team members understand social and emotional factors impacting care
- Quality Improvement: Actively contribute to performance improvement initiatives
- Flexibility: Perform additional duties as delegated by the Clinical Supervisor
Requirements
- Master’s degree in Social Work from a CSWE-accredited program
- Minimum 1 year in healthcare; hospice experience strongly preferred
- Deep understanding of hospice philosophy and end-of-life care
- Current CPR certification required
- Valid driver’s license, auto insurance, and reliable vehicle for community visits
