Responsibilities

  • Deliver high-quality patient care by performing physical examinations, vital sign assessments, and comprehensive patient evaluations.
  • Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards.
  • Manage catheterizations, tube feedings, wound care, and sterile procedures with precision and attention to detail.
  • Utilize electronic health record (EHR) systems such as Epic or Cerner to document medical records accurately and efficiently.
  • Conduct diagnostic evaluations including specimen collection, processing, and basic medical imaging when necessary.
  • Assist with emergency response procedures such as first aid, airway management, CPR, and ventilator support in urgent situations.
  • Collaborate with multidisciplinary teams on discharge planning, case management, and patient education to ensure continuity of care.
  • Support patients with specialized needs including dementia care, Alzheimer's care, hospice services, and trauma-related conditions like Level I or II trauma center protocols.
  • Engage in patient assessment activities focusing on physiology knowledge, pain management strategies—including acute pain management—and infection control practices.
  • Maintain strict adherence to HIPAA regulations and clinical confidentiality policies while managing medical documentation and medical records.

Qualifications

  • Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction.
  • Proven experience working in diverse healthcare environments such as hospitals (including ICU or emergency medicine), nursing homes, skilled nursing facilities, or home & community care settings.
  • Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks; experience with EMR/EHR management is highly preferred.
  • Strong knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and documentation standards.
  • Experience working with geriatric patients or individuals with disabilities; familiarity with dementia care and memory support is advantageous.
  • Ability to perform venipuncture (phlebotomy), catheter care, sterile techniques including aseptic procedures, and basic life support (BLS).
  • Critical care experience such as ICU or emergency room work is desirable; familiarity with trauma protocols enhances your fit for this role.
  • Excellent communication skills for effective caregiver communication and client interaction; compassionate approach to patient-centered care.
  • Knowledge of long-term care regulations and safety standards applicable across healthcare settings.
