Revital Rehab Services is seeking a dedicated Part-Time/PRN Physical Therapist (PT) to join our care team at River Terrace, a skilled nursing and rehabilitation facility in Bluffton, IN. What You’ll Do:

• Conduct assessments and develop personalized treatment plans with short- and long-term goals

• Deliver evidence-based physical therapy services in accordance with best practices and facility guidelines

• Collaborate with an interdisciplinary team to coordinate care and optimize outcomes

• Maintain accurate, timely documentation of patient progress and adhere to productivity standards

What You’ll Bring:

• Active Physical Therapist license in the state of Indiana

• Current BLS certification

• Passion for patient-centered, high-quality rehab care
