Bilingual SCN Screener and Navigator (English/Arabic)

Job Overview

We are seeking a dedicated and compassionate Bilingual SCN Screener and Navigator (English/Arabic) to join our healthcare team and support members of the Muslim community. In this important role, you will serve as the first point of contact for individuals seeking support through SCN services, guiding members through the enrollment process, identifying their health and social needs, and connecting them with available resources.

The ideal candidate is fluent in English and Arabic, culturally sensitive, and passionate about helping individuals and families access the services they need. This position requires strong communication skills, attention to detail, and the ability to build trust with members while coordinating care with healthcare providers, care managers, and community partners.

Responsibilities

• Conduct intake interviews and complete eligibility screenings for SCN services.
• Communicate with members in English and Arabic to gather required demographic, medical, insurance, and social needs information.
• Educate members on available SCN benefits and services.
• Identify barriers to care and connect members with appropriate resources, including groceries, healthy meals, kitchen supplies, nutritional counseling, transportation, and other approved supports.
• Maintain accurate documentation in Unite Us and other required systems.
• Follow up with members to complete enrollment and address barriers to care.
• Collaborate with healthcare providers, care managers, and community partners.
• Provide culturally appropriate support to members and their families.
• Ensure compliance with SCN, Medicaid, HIPAA, and company policies.

Pay: $23.00 - $25.00 per hour
