What You’ll Do
Conduct health assessments and ongoing monitoring for the people we support
Coordinate and schedule medical appointments and follow-ups
Partner with our Medical Services Team to ensure seamless, high-quality care
Collaborate with interdisciplinary teams to support person-centered outcomes
Advocate for and promote the overall health, wellness, and independence of the people we support
What You Bring
Active, valid, NYSÂ RN license
Minimum 2 years of nursing experience
Strong organizational and coordination skills
Passion for community-based care and person-centered support
OPWDD experience is a major plus
