🏃‍♂️ Your Role: Home Health Physical Therapist

As a Physical Therapist in a home health setting, you'll work one-on-one with patients to restore mobility, reduce pain, and prevent falls, empowering them to live safely and independently at home.

🛠️ Key Responsibilities:

• Conduct in-home PT evaluations and follow-up visits

• Design and implement individualized rehabilitation and treatment plans

• Improve patients’ strength, balance, mobility, and functional independence

• Educate patients and caregivers on safe movement and equipment use

• Document visits using WellSky Kinnser EMR

• Coordinate care with physicians, RNs, case managers, and the interdisciplinary team

🧠 What We’re Looking For:

• Arizona-licensed Physical Therapist (PT)

• CPR certification (AHA-approved)

• At least 1 year of clinical experience (Home Health experience preferred)

• Organized, compassionate, and reliable

• Valid driver’s license, auto insurance, and reliable transportation

• Strong communication skills and clinical decision-making
