Your Role as a PT

• Deliver personalized in-home therapy sessions tailored to each patient’s needs and goals.

• Develop treatment plans that improve strength, balance, and mobility, reducing fall risks and promoting independence.

• Educate patients and families on safe movement techniques and daily living activities.

• Document patient progress and care using WellSky/Kinnser EMR.

• Collaborate closely with physicians, case managers, and other care providers to ensure seamless care.

What We're Looking For

• Licensed Physical Therapist in California (required).

• AHA CPR Certification.

• Minimum 1 year of experience. Home Health experience is a plus but not required.

• Reliable transportation, valid driver’s license, and auto insurance.

• Strong communication, organization, and problem-solving skills.

• Compassionate, personable, and self-motivated.
