Your Role: Home Health LVN

As a Home Health Licensed Vocational Nurse (LVN), you will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). This role is ideal for LVNs experienced or interested in home health, geriatric care, dementia care, and community-based nursing.

 Primary Responsibilities

• Provide skilled home health nursing care based on individualized care plans
• Deliver care to geriatric and senior patients in home-based and assisted living settings
• Accurately document visits, treatments, and assessments in the EMR system
• Collaborate with physicians, RNs, families, and interdisciplinary care teams
• Support patients with chronic conditions, recovery, and dementia care needs
• Ensure compliance with California home health regulations and company policies
• Promote patient independence, safety, and overall well-being

Qualifications

• Active California Licensed Vocational Nurse (LVN) license in good standing
• Graduate of an accredited vocational nursing program
• Minimum 1 year of nursing experience (Home Health experience preferred)
• Experience with geriatric patients, senior care, or dementia care preferred
• Valid driver’s license, reliable transportation, and auto insurance
• Strong communication, time management, and documentation skills
• Comfortable using electronic medical records (EMR systems)
