JOB SUMMARY
The Resource Navigator acts as both an employee coach and case manager, supporting employees who identify as having a disability. This role focuses on providing individualized case management and connecting employees to internal and external resources, fostering workplace success and personal growth.
ESSENTIAL FUNCTIONS
1. Intake & Assessment – 20%
- Conduct intake assessments to identify employee strengths, needs, and goals.
- Gather information to inform individualized service planning and support strategies.
2. Service Planning & Case Management – 20%
- Develop and manage individualized service plans to support employee success at work and in life.
- Provide ongoing case management and adjust plans as needed.
3. Resource Navigation – 15%
- Connect employees to appropriate internal and community resources, including housing, healthcare, benefits, education, and mental health services.
- Ensure timely referrals and follow-ups to reduce barriers and stressors.
4. Advocacy – 10%
- Advocate for employee rights and needs within the workplace and community.
- Work to remove barriers that impact employee stability, inclusion, and success.
5. Skill Development & Guidance – 10%
- Provide guidance, workshops, and referrals that promote self-sufficiency and employment stability.
- Support employees in building coping strategies, confidence, and workplace success skills.
6. Documentation & Compliance – 10%
- Maintain accurate case notes, documentation, and follow-up assessments.
- Ensure all records meet organizational and compliance requirements.
7. Collaboration – 7%
- Work with internal teams, supervisors, and partner agencies to ensure continuity and quality of services.
- Build strong relationships that enhance support and resource access.
8. Outreach & Awareness – 5%
- Lead outreach and resource awareness activities that promote trust and inclusion.
- Share information with employees and supervisors to strengthen accessibility.
- Support community events and accessibility partnerships, including travel to conferences to provide Accessibility Services and represent Relay. Responsibilities include:
- Staffing accessibility stations at sponsored events.
- Managing and distributing sensory bags.
- Coordinating sensory break rooms.
- Overseeing wheelchair-reserved seating.
- Providing and supporting assisted listening devices.
9. Data & Reporting – 2%
- Track service utilization, outcomes, and trends.
- Prepare reports as required to support program monitoring and improvement.
10. Other Duties as Assigned – 1%
- Carry out additional responsibilities as requested to meet departmental and organizational needs.
QUALIFICATIONS
- High school diploma or equivalent; additional training in social services, case management, or disability support preferred.
- Minimum two years of experience in social service or disability support settings.
- Ability to work effectively with diverse populations and communicate across backgrounds.
- Strong organizational, documentation, and time-management skills.
- Familiarity with benefits navigation, community services, and trauma-informed approaches.
- Computer proficiency, including Microsoft Office Suite.
- Valid driver's license and insurability, as travel within Oregon may be required.
