About The Position
The Community Health Coordinator assists Reid Health achieve its mission to lead our communities to well-being, one person at a time by effectively developing, coordinating, implementing activities and outreach initiatives that align with the Core Public Health Services identified by the Wayne County Health Department the Community Health Needs Assessment. The Community Health Coordinator is primarily responsible for providing administrative support to the director, team and community through coordinating outreach and education. This position requires commitment to the overall improvement of the health of the community.

Overview Of Responsibilities
- Assists with answering the department phone and coordinating follow-up
- Develops and maintains a calendar of community events
- Completes requisitions, purchase orders and check requests
- Coordinates and assists with preparing health education sessions, workshops, and presentations to individuals and groups within the community.
- Assists with event/activity promotion by distributing flyers both electronically and in person to community partners
- Connects community members to relevant health resources and services, such as healthcare providers, clinics, support groups, and social services. (If approached when in the community)
- Provides guidance and support to individuals seeking assistance with accessing healthcare and navigating the healthcare system. (If approached when in the community)
- Evaluates the effectiveness of health education programs and initiatives through outcome assessment, participant feedback, and data analysis. Monitoring program activities and outcomes to track progress toward established goals, objectives and departmental KPIs.
- Collaborates with community stakeholders, including healthcare providers, schools, faith-based organizations, businesses, and local government agencies, to leverage resources and support collective efforts to improve community health.
- Works at the direction of the Director to address core public health services and contract deliverables as defined in the Wayne County Health Department Health First Indiana contract
- Assists Director in managing and implementing Reid’s Community Health Needs Assessment initiatives
- Tracks and reports deliverables as required for Health First Indiana grant reporting and Community Benefit reporting.
Education & Experience
- Education Required: High school diploma or equivalent
- Education Preferred: Some college
- Experience Required: Experience working in the community. Knowledge of community resources and social support services are available to individuals and families. Computer literacy required including Microsoft Office applications (Word, Excel, Outlook, PowerPoint)
- Experience Preferred: Knowledge and experience in the areas of community health, social services, healthcare settings.
