Rehabilitation Support Services, Inc. is hiring a Case Coordinator for its Certified 96 bed Apartment Program. This program serves individuals with mental health and substance use disorders. Hours:
- 40 hour work week: 8am-4:30pm (30 minute unpaid lunch)
- Sunday-Thursday (2 openings)
- Tuesday-Saturday (2 openings)
- Monday-Friday (3 openings)

Requirements for the Case Coordinator:
- A high school diploma or equivalent
- Valid NYS Driver’s License required
Responsibilities for the Case Coordinator include:
- Responsible for a designated case load; including the accurate and timely completion of case record documentation in the Agency electronic health record.
- Establish and maintain good rapport and therapeutic relationship with residents.
- Aid and encourage clients to meet the established goals of their service plans.
- Teach, monitor and evaluate client’s skills of daily living.
- Monitor and Supervise medications with clients.
- Meet with clients at least 5x per month and provide restorative services.
- Assist with transportation as needed.
- Hold a caseload of approximately 10-12 clients.
- Provide compassionate and person centered services.
- Weekly on-call rotation approximately once every three months.
