What are the responsibilities and job description for the Care Manager position at Rehabilitation Support Services, Inc.?
Rehabilitation Support Services' Care Management Program currently has an opening for a Health Home Plus Care Manager located in Schenectady County. The schedule for this position is Monday through Friday 8am-4:30pm with some flexibility.
The primary role of the Care Manager is to provide intensive Health Home services to a caseload of 15 individuals with the goal of reducing the rate of hospitalizations through engagement in all necessary community-based resources and services, and behavioral and physical health care. The Care Manager will work closely with community providers to ensure appropriate delivery of services to meet the needs of each participant. The Care Manager will conduct various assessments to assist in identifying client needs and create individualized care plans to reflect these needs.
Qualifications:
- Bachelor’s Degree in Human Services or a related field or a CASAC along with 2 years of experience in the Human Services field; or one year of experience in Health Home Care Management
- Must have strong written, verbal and computer skills
- Must be extremely organized and demonstrate the ability to carry out multiple ongoing tasks with the ability to prioritize
- Must be able to work independently and be highly dependable
