Rehab Strategies is expanding throughout Indiana and seeking a Physical Therapist (PT) to join our growing rehabilitation team.
This position offers PRN flexibility with the potential for consistent or full-time hours, making it a great opportunity whether you're looking for supplemental income or a more substantial weekly schedule.
You'll work directly with patients in a skilled nursing and rehabilitation environment, helping them regain strength, mobility, independence, and confidence following illness, injury, hospitalization, or surgery.
Responsibilities:
- Evaluate patients and develop individualized physical therapy plans of care
- Provide skilled treatment to improve mobility, strength, balance, endurance, and functional independence
- Address gait, transfer, and mobility limitations
- Recommend and train patients on appropriate assistive devices
- Establish measurable functional goals and monitor patient progress
- Modify treatment plans based on patient response and clinical needs
- Complete accurate and timely clinical documentation
- Collaborate with physicians, nursing, OT, SLP, families, and other members of the interdisciplinary team
- Provide patient and caregiver education
- Participate in discharge planning and recommendations for continued care
Qualifications:
- Graduate of an accredited Physical Therapy program
- Active Indiana Physical Therapist license or ability to obtain
- Skilled nursing, post-acute, rehab, or geriatric experience preferred
- Strong clinical judgment and communication skills
- Ability to work independently while collaborating with an interdisciplinary team
