CASE ASSISTANT - SUPPORTING THE WAIVER DEPARTMENT

SUMMARY: The Case Assistant provides support to the Clinical Staff assisting with various tasks and projects.


REQUIRED DUTIES AND RESPONSIBILITIES:

  • Fax physician office or other providers as needed to request documentation
  • Provide assistance to Support Coordinators on follow up paperwork from Medication Reconciliation
  • Coordinate Non-Emergency Medical Transportation scheduling and setup
  • Order services through Service Authorizations and setup actual service at direction of the SC
  • Updated information on Participants admission and discharge dates from hospitals and Nursing Homes
  • Assist with Self Determination process
  • Assist with various surveys as needed
  • Assist with 30 day and 2 week contacts for participants, providing clear communication to SCs on follow up needs
  • Obtain Bids on home modification needs
  • Verify and assure 3rd part insurance information
  • Collaborate with Hospice and Skilled Care as needed
  • Investigate community resources and assist participants in obtaining (example Rural Home Development/SER (state emergency relief)/United Way)
  • Request extra physician documentation for door 3,4,5s as needed
  • Provide clerical support for Support Coordinator teams
  • Schedule assessments if needed
  • Assist with transfer summaries when there is a change in participants Support Coordinator
  • Conduct home visits related to Provider audits
  • Perform other duties as assigned by the supervisor or CEO consistent with the employees skills and Agency needs
  • Enter Service Authorizations into billing program

Education and/or Experience:

  • Minimum of an Associate's degree in a social services field is preferred.
  • Experience in the health care system and the field of aging.
  • Scheduling experience.
  • Strong computer fundamentals including proficiency with Microsoft Office products.

Language Skills: Possess the ability to read, analyze and interpret information. Possess the ability to respond to inquiries or complaints from staff and vendors. Possess the ability to effectively present information to management and staff.


Mathematical Skills: Possess the ability to work with mathematical concepts and apply concepts such as fractions, percentages, ratios and proportions to work situations.


Reasoning Ability: Possess the ability to define problems, collect data, establish facts and draw valid conclusions based on information.


Other Skills & Abilities:

  • Manage time and work well with people on an individual basis.
  • Organize and access office files.
  • Accurately enter and retrieve information from a computer.
  • Sit for long periods of time.
  • Must maintain confidentiality regarding participant information.
  • Detail oriented regarding numbers and able to perform job duties with accuracy.
  • Must have experience working with computers and learning new software programs.
  • Possess effective communication skills.
  • Work as part of a team

While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to sit and use hands and fingers, handle or use objects, tools or controls. The employee is occasionally required to stand and walk.


The employee will have to occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision, distance vision and the ability to adjust focus.
