JOB TITLE: Case Manager/Supports Coordinator
JOB SUMMARY: A Case Manager is directly accountable to the Case Manager Supervisor in assisting with case management by developing supports for consumers with identified needs, coordinating a variety of available services necessary to maintain independent living, and when possible, self-sufficiency and family stabilization.
This position is also subject to criminal and abuse background checks.
Duties and responsibilities include, but are not limited to:
- Manage a caseload of clients, and maintain case files.
- Assist consumers, guardians, and families in maximizing their abilities for self-‐determination by enabling them and empowering them in decision-‐making to the greatest extent possible.
- Assist in accessing community resources identified for the consumer, family members, and/or providers.
- Maintain the designated number of contact hours.
- Maintain professional relationships with department personnel, county personnel, consumers, families, and service providers.
- Provide supportive counseling and advocacy for consumers.
- Conduct crisis intervention as necessary.
- Flexibility to adjust schedule occasionally in order to accommodate other’s schedules.
- Monitor consumers use of medications.
- Other duties as assigned by the Case Manager Supervisor.
KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS
- Strong understanding of case management principles, procedures,and practices.
- Experience with Person-Centered Treatment Planning.
- Ability and desire to work with people from various backgrounds providing basic human needs with compassion and a dignified environment.
- Experiences in crisis intervention principles and practices.
- Demonstrated ability to perform a variety of duties, occasionally changing from one task to another of a different nature without loss of efficiency or composure.
- Demonstrated computer skills including word-‐processing.
- Ability to present ideas effectively, in both oral and written form.
- Ability to negotiate and resolve conflict between dissenting parties.
- Able to lift 25 lbs.
- Strong oral and written communication skills.
- Computer skills including spreadsheets and word-‐processing.
- Strong time management.
- Be honest and trustworthy.
- Demonstrate sound work ethics.
- Contribute to a positive workplace culture.
- Minimum of two years experience in case management services.
- Professional-level knowledge of federal and state assistance programs is helpful.

MINIMUM QUALIFICATIONS
- Bachelor’s degree in Social Work or Human Services.
- Valid Social Driver’s license.

Pay: $40,000.00 - $60,000.00 per year
